The program is delivered by Independent Pharmacies of Australia through your pharmacist. This serves as confirmation that you consent to participate.

You have the right to request access to, or correction of, any information held about you, or to file a complaint regarding a breach of the Australian Privacy Principles by contacting the privacy officer as detailed in the privacy policy.

Your continued participation in the program is also subject to the terms of use, which can be found here. Information provided by you, or your healthcare professionals, will be used by Independent Pharmacies Australia, your pharmacist, or authorised program partners to deliver the program. This may include SMS, phone, or email support, which you can opt out of at any time. Occasionally, Independent Pharmacies of Australia or an authorised partner may need to contact you for program-related purposes, including but not limited to:

  • Reporting and follow-up on an adverse event
  • Changes to program terms and conditions
  • General feedback and satisfaction with the program

No identifiable personal information will be disclosed to any third party or the manufacturer of products, apart for the requirement to provide the agreed services to you, without your consent. Please note that even if you do not consent to the use of your personal information for the program, Independent Pharmacies of Australia is legally obligated to report details of any adverse events related to the testing to the supplier. If you do not consent to the use of your identifiable information, partially de-identified data, such as your initials, will be used instead.

I agree to participate in the service or program based on my own decision.

I understand my suitability and/or eligibility for the service or program will be assessed in-pharmacy prior to commencement, and if I am not suitable or eligible, I may be unable to participate.

I acknowledge that unless otherwise advised, it is my responsibility to provide a copy of any report or assessment provided in connection with the service or program to my health care professional following a recommendation by the pharmacy or any service provider in connection with the service or program.

I acknowledge that where necessary for the facilitation of the service or program, Independent Pharmacies of Australia may disclose details of my enrolment, results or other information regarding my participation in the service or program to my health care professional and/or any service provider in connection with the service or program.

I acknowledge the service or program and any information or reports generated by Independent Pharmacies of Australia or any service provider in connection with the service or program do not take into account my full medical history and should not be interpreted as a substitute for medical consultation, evaluation or treatment by a qualified doctor. For these reasons, to the maximum extent permitted by law, Independent Pharmacies of Australia or any service provider in connection with the service or program, will not be liable for the death of or injury to any person or any other loss or damage resulting from undertaking the service or program or any reliance upon the information, reports or views generated from the services or programs.

I acknowledge that if I have an existing health condition or any concerns about my health, I will obtain advice from a qualified doctor before undertaking the service or program and advise the pharmacy about the existing health condition.

While our tests are designed to provide accurate results, no test is 100% accurate. False positive and false negative results are possible. Factors such as sample quality, handling, and individual variations can affect test results. Please consult with a healthcare professional for any medical concerns or questions about your test results.

Any reference to Independent Pharmacies of Australia, as used throughout these Terms and Conditions, includes any pharmacy or other healthcare provider that is contracted by Independent Pharmacies of Australia to provide testing services.

The Know Your Numbers Health Check is a pharmacist-led program that aims to assist you with the testing of HbA1c and/or lipid levels.

I am 18 years of age or older and am the person participating in the program.

COVID-19, Influenza and Strep A Testing

The COVID-19, Influenza and Strep testing program is a pharmacist-led program that aims to assist you with the testing for one or all of the following: COVID-19, Influenza, and Strep.

I am 18 years of age or older and am the person participating in the program or for participants who are under the age of 18 years I am the parent or guardian of the participating child.

I confirm that I consent to participate in the program in accordance with the above.
